- If necessary Quadrant will contact the veterinary surgeon to discuss treatment and
obtain copies of required reports.
- Insured. must complete a claim form if animal dies & provide a post mortem report.(see
aboce for downloading forms).
The essential procedures to ensure you have a trouble free settlement are:
Do...
- Call Quadrant when an animal is injured or becomes sick. 021 765 406 (24-hour Claim hotline).
- Report a loss immediately to Quadrant.
- Make arrangements for an autopsy/post mortem. A post mortem is obligatory.
- Advise anyone having care, custody and control of the animal of these procedures.
Don't...
- Have any animal operated on without the permission of Quadrant.
- Put any animal down without permission of Quadrant unless on humane grounds as imperative to terminate excessive incurable suffering.
- Have the animal removed prior to the performance of an autopsy/post mortem.
- Place any other insurance on the animal without advising Quadrant.
